Abstract
Aim To investigate the relationship between the 27 base pair(bp) insertion/deletion(a/b) polymorphism of 4th intervening sequence in endothelial nitricoxide synthase(eNOS) and NO level in diabetic nephropathy's patients for acknowledging the effect of genetic factors on diabetic nephropathy(DN) .Methods We examined 37 healthy controls and 84 cases with type 2 diabetes mellitus(DM).Type 2 DM cases were divided into three groups according to UAER.(1)The normal albuminuria group(DM1 group) ;(2)The trace albuminuria group(DM2) group ;(3)The mass albuminuria group(DM3) group .A 27 bp variable number of tandem repeats (VNTR) polymorphism of eNOS was identified by polymerase chain reaction(PCR).Fasting serum nitricoxide metabolites(Nox) of all the patients were measured by nitrate reductase.Results (1)The frequency of a allele and aa+ ab genotype in all type 2 DM group were significantly higher than that in control group(χ 2=3.91 to 4.28,P< 0.05;χ 2=17.16 to 36.05,P< 0.01).The frequency of a allele and aa+ ab genotype in DM1 group were significantly lower than that in DM2 and DM3 group(χ 2=4.17 to 4.64,P< 0.05;χ 2=8.23 to 9.21,P< 0.01).(2)Fasting serum Nox in aa+ ab genotype were significantly lower than that in bb genotype of normal control(NC) group(t=2.30,P< 0.05).(3)Serum Nox in DM1 was significantly higher than NC(u=2.35,P< 0.05),but the NO levels of DM2 and DM3 were significantly lower than those in NC group(u=1.68,P >0.05).Conclusion The polymorphism of eNOS gene 27 bp VNTR is associated with NO level in DM patients.These results supported that eNOS gene would lead to the progress of DN by decreasing NO level in DM patients,thus the quality of life of patients is improved.关键词
