Abstract

BACKGROUND: Transforming group factor-beta (TGF-β) is a clan of protein peptide with multiple functions, of which, TGF-beta1 plays the main role in union of bone fracture. The interfixation of various materials affects fracture union in different ways. The degree of influence is the important index to assay the quality of interfixation materials.OBJECTIVE: To understand intracellular orientation of TGFβ1mRNA by the expression of it at various phases of bone fracture union after interfixation with different materials so as to probe into the effects of different interfixation materials on the local expression of TGFβ1mRNA during fracture union.DESIGN: Randomized control observation was designed.SETTING: Department of Orthopedics and Department of Pathology of Changhai Hospital, Second Military Medical University of Chinese PLA.MATERIALS: The experiment was performed in Experimental Animal Center and Laboratory of Department of Orthopedics of the Second Military Medical University of Chinese PLA from March 2000 to July 2001, in which, 42 New Zealand rabbits were employed of either sex and randomized into two groups based on fixed materials, named as the group of rectangle-shaped intramedullary nail (RIN group) and group of stainless steel plate (SPL group), 21 rabbits in each one.METHODS: Interfixation model was prepared on rabbit tibia with either RIN or SPL, 2 mm inferior to the conjunction of tibia and fibula on the left side. Before and after operation, the rabbits were bred with market-sold animal granular forage in separate cages and they were free of drinking pipe water and movement, without external fixation. Three rabbits were sacrificed in 1, 3, 7, 14, 21 and 28 days after interfixation in each group successively. The callus on fractured end and peripheral tissue specimen were collected for managements of tissue slices and in situ hybridization.MAIN OUTCOME MEASURES: Positive staining intensity and positive area ratio of TGF β1mRNA in different cells were observed and recorded in tissue slices and the distribution and contents of TGF β1mRNA in callus during fracture union in rabbits were displayed.RESULTS: Totally 42 animals were employed totally in the experiment, in which, 3 rabbits were died from diarrhea and 3 rabbits were infectious in local wound and had bone exposed. Totally 6 rabbits had not included in result analysis, 3 rabbits in each RIN group and SPL group and 36 rabbits sion of TGF β1mRNA inlocal cells and substances in both RIN and SPL groups. Seven days later, TGF β1mRNA was expressed positive in mesty, positive area ratio and positive index of TGF β1mRNA in both RIN and SPL groups were the highest on the 14th day after operation (12.36±0.58,0.19±0.05,2.35±0.20; 10.24±0.65,0.13±0.03,1.33±0.10) and were the lowest on the 28th day (4.24±0.57,0.06±0.02,0.25±0.08 ;3.31±0.27,0.03±0.01,0.10±0.02). With the comparison at same phase, except positive area ratio on the 7th day, the rest items in RIN group were all higher than SPL group and the difference among groups was significant (P < 0.05, P < 0.01).CONCLUSION: Rectangle-shaped intramedullary nail is more advantageous to promote secretion of TGF β1 in cells and benefits fracture union compared with stainless bone plate.
