Abstract
BACKGROUND:Magnetic resonance imaging(MRI) is a useful noninvasive modality to evaluate patients with Parkinson's disease(PD).Recent studies have suggested that white matter hyperintensities(WMH) on MRI are more likely in PD patients than in healthy subjects.However,the impact of WMH on the clinical features of PD has yet to be clarified. tures of PD. DESIGN:Randomized,blinded case-control trial design. SETTING and PARTICIPANTS:Ninety PD patients were selected from the outpatients and inpatients of Neurology Department of Xijing Hospital from January 1996 to June 2003.Sixty age-matched healthy volunteers were also recruited as controls. INTERVENTIONS:The frequency and extent of WMH on MRI between patients with PD and healthy controls were analyzed and compared,and the correlation of WMH with the clinical features of PD was examined independently by two experienced radiologists. MAIN OUTCOME MEASURES:The frequency and degree of WMH,including periventricular hyperintensities(PVH) and centrum semiovale hyperintensities(CSH),on MRI in patients with PD and healthy controls were observed.The clinical features of PD including disease duration,disease severity(assessed using Hoehn-Yahr stage),and disease progression index(ratio of the Hoehn-Yahr stage to disease duration) were evaluated. RESULTS:The frequency of PVH was significantly higher in PD patients than in healthy controls(40% vs 22% ,χ 2=6.125).PVH was significantly severer in PD patients than in healthy controls(1.81± 0.52 vs 1.23± 0.60 ;t=3.333),and the patients with PVH had significantly shorter disease duration than those without[(6.11± 2.31) years vs (7.81± 2.50) years,t=3.278],having also greater disease severity(Hoehn-Yahr stage 2.68± 0.92 vs 1.83± 0.94,t=4.325),and higher disease progression index(0.46± 0.26 vs 0.31± 0.21,t=3.038).However, the frequency and degree of CSH did not significantly differ between patients and the controls,nor did the disease duration,disease severity,or disease progression index between PD patients with and without CSH. CONCLUSIONS:PVH may influence some clinical features of PD, and reflect a more rapid neurodegenerative process of PD. 关键词
