AG与潜在HCO3-在呼吸衰竭急性加重期酸碱失衡诊断价值的探讨
Anion gap and potential HCO3- acid-base disorder diagnosisvalue in patients with acute period of chronic respiratory failure

Abstract
Objective: To detrmine the role of acid-base disorder in diagnosis of acute period of chronic respiratory failure.Method: 40 patients, suffered form chronic pulmonary disease with chronic respiratory failure, were selected. The arterial blood and electrolytes were determined synchronically from a total of 119cases. The expected compensatory formulas of acid-base disturbance were combined with the anion gap (AG) and the potential HCO3- to determine the kinds of acid-base disorders.Results: More than 95% of the total cases tested showed acid-base disorders. Among them,about 37.8%(45 cases) and 50.4%(60 cases) and 7.6%(9 cases) were single-, double-, and triple-acid-base disorders, respectively. Only 4.2%(5 cases) of the total cases were without any acid-base disorders. If AG was determined, the number of cases diagnosed as the metabolic and the triple acid-base disorders were increased from 8(6.7%) to 17(14.3%), and from 0(0%) to 9(7.6%), respectively. Similarly, with HCO3- the percent of the triple acid-base disorders detected were increased from 4.2%(5 cases) to 7.6%(9 cases). Conclusions: ①Acid-base disorders appears to be associated with chronic respiratory failure from chronic pulmonary diseases;②the AG value and potential HCO3- were useful in the diagnosis of metabolic acidosis and triple acid-base disturbance.关键词
