摘要

Abstract

Aim To investigate the relationship between serum resistive level and leptin level, and compare serum resistive level and leptin level with plasma glucose,insulin resistance and β-cell secretory function. Methods Oral glucose tolerance test and insulin release test were conducted to measure plasm glucose concentration and serum insulin concentration for calculating glucose area (AUCG) and insulin area (AUGIN) under the curve, the radio of changes in glucose and insulin within 30min after glucose ingestion (Δ I30/Δ G30),and insulin sensitivity index(ISI) by Cederholm formula among seventy-one hypertensive patients,33 males,38 females,aged 37- 69,who sought medical advice in Shanghai Changhai Hospital 2000-10/2001-12. According to the results,71 patients were divided into 3 groups:type 2 diabetes mellitus group(DM group),impaired glucose tolerance group (IGT group), and normal glucose tolerance group(NGT group).Then fasting serum concentrations of resistin and leptin were measured in three groups. Results Fasting serum resistin concentration in DM was( 30± 12)μ g/L,significantly higher than that in NGT[(22± 8)μ g/L,P< 0.05], appreciably higher than that in IGT[(25± 10)μ g/L,P >0.05].Pearson analysis showed that fasting serum resistin concentrations were highly positively correlated with AUCG( r=0.380, P< 0.001) , and highly negatively correlated with ISI(r=- 0.276,P< 0.05) and Δ I30/Δ G30( r=- 0.298,P< 0.05). Fasting serum leptin concentration in DM was( 22.2± 10.9)μ g/L,significantly higher than that in IGT[(12± 9)μ g/L,P< 0.05] and NGT [(10± 7)μ g/L,P< 0.05]. Pearson analysis shows that fasting serum leptin concentrations were positively correlated with AUCG( r=0.442,P< 0.001) ,negatively correlated with ISI(r=- 0.468,P< 0.001),but not correlated to Δ I30/Δ G30( r=- 0.111,P >0.05) . Fasting serum resistin concentrations were highly positively correlated with fasting serum leptin concentrations( r=0.253,P< 0.05) .A stepwise multiple regression analysis showed that resistin level had an important effect on AUCG and leptin had not a direct impact on AUCG.Conclusion Resistin and leptin effect energy metabolism and balance, but resistin is more closely correlated with glucose metabolism than leptin in essential hypertension patients.
