低浓度七氟醚复合全麻对上腹部手术应激反应的影响

Abstract
Objective To study the effects of combination of sevoflurane inhalation and epidural anes-thesia on stress response during operation on epigastric region .Methods 20 patients (ASAⅠ～Ⅱ) undergoing elective operation on epigastric region were enrolled in this study. ECG, SPO2 and BP were monitored throughout anesthesia. Puncture of center vein and puncture of radial artery were performed before induction of anesthesia. Anesthesia was induced with fentanyl 0.03mg/kg and propofol 2mg/kg.Intubation was facilitated with atracurium 0.5mg/kg when patients lost consciousness. MAP, HR and BIS were continuously monitored during operation.Blood samples were raked before induction of anesthesia, after intubation, after skin incision , 30min after skin incision and at the end of operation for determination of Cor, PRA and Angiotensin Ⅱ concentration.Results Compared with the baselines after anesthesia , SBP, MAP, HR and COR decreased significantly and PRA and Angiotensin Ⅱ are no significant change. Conclusion Sevoflurane inhalation combined with epidural anesthesia has the advantages of fastest recovery , increased safety and decreased stress in effect.关键词
