Impact of bone polypeptide on the promotion of new bone formation during the restoration of skull defect

Abstract
BACKGROUND: The restoration of skull defect usually uses artificial materials, fresh bone autografting, and skull autografting at present. The impact of bone polypeptide on the promotion of new bone formation in the restoration of skull defect deserves thorough research.OBJECTIVE: To investigate the impact of bone polypeptide ghrelin on the promotion of new bone formation during the in situ restoration of skull defect by in vitro skull flap.DESIGN: A case-control study based on diagnosis.SETTINGS and PARTICIPANTS: Forty-five patients of craniocerebral injury required operations to remove bone flap for decompression were selected form the first affiliated hospital of the Faculty of Medicine of Shantou University from June 1996 to December 2002. There were 24 patients in experimental group and 21 patients in control group. There were 48 skull defects, of which there were 26 defects in the experimental group and 22 defects in the control group. All patients were aged from 5 to 63 years old.INTERVENTION: Forty-five patients including 48 defects received a secondary phase restoration in situ by in vitro skull flap, of which patients in study group received a restoration by in vitro porous skull flap +bone polypeptide ghrelin, while cases in control group received a restoration by in vitro porous skull flap.MAIN OUTCOME MEASURES: Complications, bone flap absorption and bone regeneration in both groups.RESULTS: The bone flap site did not mobilize but with perfect appearance. The image examination revealed that the apposition was good at the conjugation point with gradually disappeared gap and the formation of callus could be seen. And the density of bone flap was normal in the experimental group; however, there were 5 bone flaps had decreased density in central part with some bone absorption in the control group. The comparison between study and control groups had significance ( P < 0. 05 ).CONCLUSION: The local application of bone polypeptide ghrelin can prevent bone flap absorption and promote new bone formation.关键词
