Abstract
BACKGROUND: Effect of atorvastatin therapy on hyperuricemia in Tibetan patients with hyperuricemia combining with hypercholesterolemia has not been fully understood up to now.OBJECTIVE: To investigate the association between serum uric acid (UA) and carotid intima-media thickness (IMT) and the effect of atorvastatin therapy on Tibetan patients with hyperuricemia combining with hypercholesterolemia.DESIGN: Before-after controlled study.SETTING: Department of Cardiology, General Hospital of Chinese PLA; Department of Cardiology, General Hospital of Tibetan Area Command of Chinese PLA.PARTTCIPANTS: The study was done at the Department of Cardiology, General Hospital of Tibetan Area Command of Chinese PLA from August 2005 to August 2006. Totally 80 in-patients aged 45-61 (mean 52.1 ±9.7) years with hyperuricemia and dyslipidemia were involved in this study. All patients provided the confirmed consent.METHODS: ① All participants received 20 mg/d atorvastatin in a single nocturnal dose for 12 weeks. ② Venous blood was sampled for the measurement of uric acid (UA), total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), blood urea nitrogen (BUN), creatinine (Cr),glutamic-pyruvic transaminase (GPT) aminotransferase at baseline and 12 weeks after treatment. Carotid intima-media thickness (IMT) was detected by B-mode ultrasound.MAIN OUTCOME MEASURES: ① The lipids, UA, BUN, and Cr levels; ② the association between risk factors of cardiovascular disease and IMT.RESULTS: A total of the 80 patients were involved in the final analysis. ① After administration of atorvastatin, levels of TC, TG, LDL-C and UA were decreased 33.8%, 24.9%, 12.2% and 12.6% as compared with those before treatment,respectively, and the differences between after and before treatment were significant (t=13.153 4, 3.837 3, 3.220 1,2.788 3, P< 0.01). However, levels of HDL-C, BUN, Cr and GPT were not changed remarkably before and after administration (P > 0.05); and this suggested that atorvastatin did not have effect on liver and renal functions. ②Multivariable regression analysis demonstrated that levels of TC, TG, LDL, HDL and UA were correlated with IMT value before administration (β value=0.178, 0.145, 0.189, -0.181, 0.284, P < 0.05-0.01). The β value of UA was especially high, which indicated that serum uric acid played an important role in carotid atherosclerosis.CONCLUSION: ① Serum uric acid may play an important role in carotid atherosclerosis. ② Atorvastatin has a favorable effect on lipid metabolism and it can decrease the serum UA level but not damage liver and renal functions.关键词
