椎间融合器治疗重度椎管狭窄重建脊柱稳定性的生物力学评价
Biomechanical evaluation of interbody fusion cages on the reconstruction of spinal stability in the treatment of severe lumbar stenosis

Abstract
BACKGROUND: Lumbar interbody fusion cage is a new surgical mode in the treatment of lumbar intervertebral canal stenosis, which in gradually applied in clinic. However, there are few evaluations regarding its biomechanics.OBJECTIVE: To investigate the merit of posterior interbody fusion cages in clinical application and to evaluate its biomechanical capability as well.DESIGN: A retrospective analysis based on the diagnosis was conducted in clinic. In laboratorial part, it was a randomized controlled study.SETTING and PARTICIPANTS: The study was conducted in the Department of Spine Surgery, the First Affiliated Hospital of Nanhua University. Thirty-seven severe vertebral canal stenosis patients including 22 males and 15 females, aged from 39 to 76 years old(a mean age of 52. 9 years old)were collected since 1996. Six fresh adult corpuses were obtained from the Department of Anatomy, Nanhua University.INTERVENTION: Posterior wide decompression and interbody fusion cages were preformed in thirty-seven cases. Biomechanical stability was evaluated by random grouping in human cadaveric functional spinal unite(FSUs).MAIN OUTCOME MEASURES: Therapeutic effect was evaluated by the criteria set up by Japanese Orthopaedic Association(JOA) . The range of motion(ROM) was observed in biomechanical evaluation.RESULTS: Thirty-three patients received postoperative follow-up after half a year and 4 missed and the follow-up rate was 34/37. The excellent and good rate was 90. 8% and the solid fusion rate was 94. 8% in 33 cases without severe complications. Biomechanical results indicated that the stability of lumbar segment was restored in interbody fusion cage group, which had significance in the comparison with entire vertebral lamina wide decompression (P < 0. 05).CONCLUSION: The application of interbody fusion cage implant method in the surgical treatment of severe vertebral canal stenosis has advantages of reconstructing stable spine, restoring intervertebral space and improving fusion rate.关键词
