Emergency contraception with multiload Cu 375 SL IUD: a multicentre clinical trial

Abstract
Objective: To evaluate the efficacy and acceptability of insertion of multiload IUD for emergency contraception.
Methods: A total of 1 013 women requesting emergency contraception were recruited. Among them 843 were parous women and 170 nulliparous women. Multiload Cu 375 SL IUD was inserted within 120 hours after unprotected intercourse. Urine test for pregnancy was performed before IUD insertion to rule out pregnancy. Subjects were followed up one week after the expected date of the next menstruation. Pregnancy test or ultrasound scanning was performed if menstruation did not return. Fourteen subjects were lost in follow-up. Efficacy of preventing unplanned pregnancy was analyzed basing on 999 cases and calculated according to Dixon's method. Efficacy and side effects were compared between the parous and nulliparous groups.
Results: Reasons for requesting emergency contraception of these subjects were mostly due to contraceptive failure, such as condom breakage, withdrawal failure, accounting for 54.7%. No contraception accounted for 45.3%. There were two pregnancies, one in each group, the pregnancy rate was 0.2 per 100 women. The efficacy rate of preventing unplanned pregnancy was 97.81% in the parous group and 90.96% in the nulliparous group, the difference was not significant. The total efficacy rate was 96.48%. According to the record on the menstrual cards,644 women (65%) had had intercourse after IUD insertion. No more pregnancy was found at follow-up visits. There were 2 complete expulsion and 3 partial expulsions of IUD. Thirty-nine cases had IUD removed due to pain and bleeding, more in the nulliparous group. After the return of menstruation, 95.7% in the parous group and 80% in the nulliparous group continued using IUD for contraception. Changes in menstrual cycle and bleeding were not significant. Neither infection nor trauma has occurred.
Conclusion: IUD insertion is a safe and effective method for emergency contraception for both parous and nulliparous women. The advantages of using IUD are its long term contraceptive effect. It can be a method of choice even in nulliparous women who have previous artificial abortions. Care should be taken in prophylactic measures in women with history of reproductive tract infection.关键词
