青少年隐匿性脑血管畸形的影像学诊断与分析

Abstract
Objective To investigate value of clinical features、 CT、magneticresonance imaging(MRI)and microsurgery in diagnosis of youth with angiographically occult cerebrovascular malformation(AOVM). Methods All 25 youths with AOVM(15 with moderate headache、vomit、conscious deficit, 7 with epilepsy and hemicrania, 3 with motive and conscious deficit of extremities, and the results of cerebral digital subtractive angiography of all were negative)were examined by CT and MRI. According to the CT scan, 15 patients presented with intracerebral hematoma, 10 with mixed density(2 with calcification)and 16 with enhanced vessels by enhanced CT scan. MRI showed that mixed high、low signal was surrounded with circular low signal area or single high signal. Results All patients were operated by microsurgery. The abnormal small vessels on the wall or the border of hematomas with the brain were found in 9 cases. Pathologic results: 17 arteriovenous malformations, 8 cavernous angiomas. Conclusion The diagnosis about AOVM can be done or prompted by CT and MRI. MRI is better than CT. The clinical features can be used as the consultations of diagnosis. The microsurgery and pathology are the best methods in diagnoses of AOVM. 关键词
