糖尿病社区分级管理模式的探讨
An analysis on community-based management of diabetic patients

Abstract
Objective To explore the role of community-based management in the control of diabetes and to establish a pattern of diabetic control in Chinese population.Methods Based on the blood glucose levels,conventional or intensified management was performed in 189 diabetic patients who had been registered at three different health centers in New Gongming district and Baoan district.The health centers were responsible for the routine management while hospitals were for therapies and technical guidance.The evaluation indices of the management included the management skills of family clinicians in the centers and their will to perform the management plan,treatment compliance in the patients,and blood glucose levels.Results After management,the rates of management skill improvement,will to perform the management plan,and treatment compliance differed significantly,increasing from 75.5% to 98.0% (χ2 = 8.29,p<0.01),36.7% to 95.9% (χ2 = 39.75,P<0.01),and 26.5% to 47.6% (χ2 = 38.60,P<0.01),respectively; levels of fasting glucose declined from 6.99± 1.47 to 8.49± 3.35 mmol/L (t= 5.64,P<0.01),2-hour blood glucose from 12.67± 5.37 to 9.12± 2.75 mmol/L,(t= 8.09,P<0.01),and glycated hemoglobin from 8.45± 2.22 to 6.76± 0.72 mmol/L (t= 9.94,P<0.01).The target-reaching rates of fasting glucose,2-hour blood glucose,and glycated hemoglobin elevated markedly from 25.9% to 48.2% (χ1 = 20.49,p<0.01),20.0% to 38.5% (χ2 = 12.08,P<0.01),and from 14.8% to 40.7% (χ2 = 32.31,P <0.01).Conclusions Community-based management of diabetes is easier performed by family clinicians,has higher treatment compliance and a better glucose control,and is worth popularizing.关键词
