Abstract
Objective To investigate risk factors, clinical features and diagnostic experiences of complex neonatal single ventricle congenital heart disease, so as to provide basis for diagnosing and preventing this disease better. Methods Data of one case of newbern single atrium, single ventricle and persistent truncus arteriosus were reported and 11 cases reports of same disease seen in the past 10 years in our country were reviewed. Results The mothers of 5 cases had a determined history of abnormal pregnancy and/or history of viral infection during early pregnancy, history of exposure to toxins. 8 neonates with complex congenital heart diseases presented with dyspnea,facial cyanosis and other clinical manifestations within 24 hours after birth, and their cyanosis was difficult to be corrected by oxygen inhalation. 9 cases of complex congenital heart diseases were diagnozed with color Doppler ultrasonography, and diagnoses of 3 cases were confirmed by pathological autopsy. The prognosis of neonates with this disease was poor, 4 neonates died several days after birth, 6 neonates were given up treatment by their family members, 1 neonate improved and discharged afar treatment, only 1 neonate survived up to 11 years of age when the diagnosis was determined. Conclusion The causes of congenital heart disease are complex, at present the researchers considered preliminarily that complex congenital heart disease is related to genetic and environmental factors as well as viral infections, drugs, radiation and other related exposures. Clinical manifestations of heart failure and changes in radiogram of the neonate are not typical. Cardiac color Doppler ultrasonography is a non-invasive, rapid, highly accurate and most valuable method to early identify and diagnose congenital heart disease in neonatal period. The key measures to prevent neonatal congenital heart disease include strengthening maternal health, preventing maternal infections, strengthening prenatal examination and developing fetal imaging and fetal hemodynamic examinations.关键词
