Effects of losartan and simvastatin on urinary podocyte excretion in diabetic nephropathy patients

Abstract
objective To study the effects of losartan and simvastatin on the urinary podocyte excretion in diabetic nephropathy patients. Methods Twenty - seven type 2 diabetic nephropathy patients complicated by hypercholesterolemia were divided into two groups. Group A (15 cases) received 100 mg/d iosartan and diet control. Group B ( 15 cases) received 100 mg/d losartan and 20mg/d simvastatin and diet control. Fifteen healthy volunteers served as control group. After 6 -week washout,the patients were treated by the two different methods for three months;then pedocytes were identified and quantitated by immunofluorescence staining of urinary sediments labeled by monoclonal antibody pedocalyxin. Results The amount of podocytes in the urine of group A and group B were greater than that in normal control group. After 6 - week washout, the levels of cholesterol and blood glucose and blood pressure in group A and group B were controlled in requested range. The amount of podocytes in the urine aftertreatment were lower than the amount beforetreatment, and the amount of pedocytes in the urine of group B was lower than the amount in group A (0.87 ± O. 20:1.66 ± 0.42). Conclusions The change in amount of podocytes in the urine is a sign of pathogenetic condition change in diabetic nephropathy patients. Therapeutic combination of losartan and simvastatin decreases the levels of blood pressure and blood lipid, significantly decreases the amount of podocytes in the urine, and improves the prognosis in diabetic nephropathy patients.关键词
