多层螺旋 CT 对小肠病变的诊断价值

Abstract
Objective:To investigate the value of multi-slice CT(MSCT) and reconstruction techniques in the diagnosis of intestinal diseases. Methods:Sixty cases with small intestinal diseases underwent MSCT scan of whole abdomen. CT angiograms were reconstructed with multiplannar reconstruction (MPR), volume rendering (VR), maximum intensity projection (MIP)and thin-slab maximum intensity projection (TSMIP). Results:There were 20 cases of small intestinal tumor, including 12 cases of adenocarcinoma, with the CT signs of lobular mass and obstruction in proximal terminal. There were 8 cases of interstitialoma, in which there were large masses rich in blood supplying without obstruction, and 2 cases of lymphma, in which the intestinal wall thickened variously in different part. There were 28 cases of intestinal obstruction, in which the intestinal wall dilate obviously in proximal terminal while normal in distal terminal,and the transformation zone become gradually beak-like narrow in some cases. In all 6 cases of Crohn's diseases,the intestinal wall thickened irregularly, the whole or layered wall inhancement and the comb-like mesenteric veins could be seen. Two cases of embolism in mesenteric arteries and 4 in veins, the former could be seen the twist, thin, irregular arteries,as well as calcification plaque, and the number of branches decreased.The latter showed widely longitual filling defect,intestinal wall thickening, and the mensenterium edema. Conclusions: MSCT can display the whole small intestine and mesentery as well as mesenteric veins clearly and should be used widely in clinical practice in diagnosis of small intestine diseases.关键词
