Abstract
Objective To evaluate the effect of disease details management program for hypertension in community on awareness of hypertension related knowledge through community based multi - center trial of intervention. Methods Totally 1 442 patients from 4 communities in urban Shanghai were enrolled and divided as intervention group ( n = 961 ) and usual group ( n = 481 ). The intervention programs for the intervention group included disease management by special care managers who enhance communication between physicians and patients, promote medication compliance and give guidance of health lifestyle for patients; collect the related information on blood pressure, etc. As well as feedback from patients for community practitioners; and community-based management information platforms and patient management tool were also developed and applied. Only regular management was given for the usual group. Results The awareness of knowledge on hypertension was improved in both the intervention and usual groups. The values of systolic blood pressure ( SBP ) and diastolic blood pressure ( DBP ) of the intervention group were lower than those in the usual group; the scores of awareness of normal blood pressure standard, risk factors and prevention measures of hypertension were significantly better in the intervention group than in the usual group ( P ＜0. 05 ). Correlation coefficient of SBP with total score of awareness in the intervention group was - 0. 352 ( P = 0. 000 ), and that of DBP was -0. 262 ( P = 0. 000 ) . Conclusion Disease details management program for hypertension in Shanghai communities is effective to improve the degrees of awareness of related knowledge and control of the blood pressure.关键词
