Abstract
Objective To compare the application of Modified Early Warning Scoring ( MEWS ) system and Calibrated MEWS system in the pre - hospital emergency care. Methods A total of 300 patients who were accepted through 120 emergency care system were enrolled in this study. MEWS and calibrated MEWS were applied to assess the patients immediately after the health - care staff met the patients to guide triage. The 30 - d outcomes were regarded as the patient's prognosis. Results The distributions of MEWS scores and calibrated MEWS scores showed significant difference among patients died during emergency care ( P ＜0. 01 ) and among those who survived ( P ＜ 0. 01 ). The distributions of MEWS scores and calibrated MEWS scores showed significant difference among patients who were admitted to ICU and those who received pre - hospital first aid ( P ＜0. 01 ) but not among those who received treatment in outpatient departments ( P ＞ 0. 05 ). Calibrated MEWS scores were significantly higher than MEWS scores in both dead and survived patients ( P ＜0. 01 ) and in those who were admitted to ICU and those who received pre - hospital first aid than those who received treatment in outpatient departments ( P ＜0. 01 ). With death as the prediction indicator, the areas under ROC curve were significantly different between MEWS and calibrated MEWS scores ( P ＜0. 05 ). With admission to ICU and pre - hospital first aid as the prediction indicators, the areas under ROC curve were significantly different between MEWS and calibrated MEWS scores ( P ＜0. 01 ). Conclusion Compared with MEWS system, calibrated MEWS system can more accurately reflect the conditions of emergency patients, can be used for predicting case - mortality rate, and can be performed in an easier way. Therefore, it can be widely applied in pre- hospital first aid.关键词
