经鼻持续正压通气治疗妊娠晚期合并阻塞性睡眠呼吸暂停低通气综合征的临床研究
Nasal Continuous Positive Airway Pressure Treatment for Women with Obstructive Sleep Apnea Hypopnea Syndrome at Late Trimester of Pregnancy

Abstract
Objective To analyze the possible factors associated with the development of obstructive sleep apnea hypopnea syndrome ( OSAHS ) in women at late trimester of pregnancy and to observe the efficacy of nasal continuous positive airway pressure ( nCPAP ) treatment for their OSAHS. Methods Twenty one normal pregnant women ( control group ) and twenty four pregnant women with OSAHS ( OSAHS group ) were recruited and their age, body height, body mass index ( BMI ) and neck circumference ( NC ) were compared between the two groups. Correlation of apnea hypopnea index ( AHI ) with age, body height, BMI and NC was made in the OSAHS group, nCPAP treatment was performed on women in the OSAHS group and comparison before and after 30 days nCPAP treatment was also made as for AHI, lowest pulse oxygen saturation ( L - SaO2 ) as well as longest apnea duration. Results There were statistically significantly differences in BMI and NC ( P ＜0. 05 ) , but no statistically significantly difference was found in age and body height ( P ＞ 0. 05 ) between OSAHS group and control group. In the OSAHS group the correlation coefficient of AHI with BMl and NC were respectively 0. 603 and 0. 696 , in a positive correlation ( P ＜ 0. 01 ). After 30 days nCPAP treatment. AHI and longest apnea duration were significantly decreased ( P ＜0. 01 ), L - SaO2 was significantly increased ( P ＜0. 01 ) . Conclusion In OSAHS women at late trimester of pregnancy the age is older, and BMI, and NC are larger, and they are of good correlation with AHI. To measure the NC and BMI is simple and convenient, which can be taken as the preliminary screening indexes for predicting development of OSAHS. nCPAP treatment was effective and with an good intolerance , being worth popularization for pregnant women with OSAHS.关键词
