Abstract
Objective To investigate the predictional value of the serum PAPP-A at 12 ～ 16 weeks in hypertensive disorders in pregnancy and the relationship between the placental pathological change of hypertensive diorders in pregnancy and the serum PAPP-A at 12 ～ 16 weeks. Methods From January of 2008 to December of 2009,2025 singleton pregnancies in the Zhongshan Hospital ,Sun Yat-Sen University who came for rutine prenatal care at 12 ～ 16 weeks and visit us regular later were invoved in this study. The serum PAPP-A at 12 ～ 16 weeks were used to prospective screening for early PE (patient required dehvery before 34 weeks because of pre-eclampsia), later PE and gestational hypertention(GH).Patients who were abortion,with any medical or surgical complication,with fetal abnormality or didn't come back our department for further care were ruled out from the study. The pathology of the placenta of the patients with hypertensive diorders in pregnancy were collect, and the same number of placenta of the normal pragnancy were randomly sampled as controls. Results There were 1687 unaffacted controls, 88 with GH,34 with early PE,128 with later PE. Serum PAPPA in group early PE was decreased. In GH and later PE were not signifcantly different from controls. All the pregnancies developed hepertensive disorders were with placental pathological change. But there were different kind changes in different groups. Conclusion There were obvious placental change in hypertensive disorders in pregnancy. The decrease of PAPP-A at 12 ～ 16 weeks is relation to the early PE and it's placental change. PAPP-A at 12 ～ 16 weeks is a useful index for prediction of early PE.关键词
