Relationship between collateral circulation and neurologic impairmen in patients with unilateral middle cerebral artery stenosis or occlusion

Abstract
Objective To study the relation between collateral circulation and neurologic impairmen in patients with unilateral middle cerebral artery stenosis or occlusion.Methods Eighty-six patients with cerebral infarction of MCA territory were examined by transcranial Doppler ultrasonography ( TCD ).The National Institutes of Health Stroke Scale (NIHSS)was uesd to investigate the degree of nervous functional defects of all the patients at the admission day and at 21d after onset.Results TCD showed that 64 cases were compensated by ACA,35 easeswere compensated by PCA, and 27 cases were compensated by both ACA and PCA.RVACA in the patient group was significandy higher than that on controls(P <0.01 ); RVACA in the patients with stem and cortex infarctionwas higher than those of controls ( P < 0.01 or P < 0.05 ).The NIHSS scores of patients with both ACA and PCA compensation were significantly lower than those of patients with ACA or PCA compensation at the admission day and at 21d after onset;and the NIHSS scores ofACA compensated patients were significantly lower than those of PCA compensated patients ( all P <0.01 ).Conclusions The higher degree of MCA stenosis is,the higher the frequency of collateral circulation compensatory is, the higher level of the collateral circulation opening up is.The disease condition and prognosis in patienta with enough collateral circulation are comparatively better.关键词
