Abstract
Objective To investigate clinical significance of hyaluronic acid(HA) and collagen type Ⅳ in gastric juice and serum in differential diagnosis between benign and malignant ulcer. Methods Contents of HA and collagen type Ⅳ in gastric juice and serum were determined by radioimmunoassay in 34 patients with ulcerated gastric carcinoma,20 patients with gastric ulcer,while 30 patients with superficial gastritis as controls. Results Levels of HA and collagen type Ⅳ in gastric juice were significantly higher in patients with gastric carcinoma than those in patients with gastric ulcer and superficial gastritis(P ＜0. 05, ＜0. 01 ). Content of serum HA was also significantly higher in patients with gastric carcinoma than that in patients with gastric ulcer and superficial gastritis( P ＜0. 05,P ＜ 0. 01 ). Concentration of serum collagen type Ⅳ was significantly higher in patients with gastric carcinoma than that in patients with superficial gastritis(P ＜0. 01 ). There were significant correlations between concentrations of HA and collagen type Ⅳ in gastric juice and serum(P ＜0. 01 ). The diagnostic sensitivity of HA and collagen type Ⅳ in gastric juice for gastric cancer was 79. 41％ and 61.76％ respectively and the sensitivity of conbined detection of HA and collagen type Ⅳ was 88.24％. Conclusion Detecting the levels of HA and collagen type Ⅳ in gastric juice may be as an aid in differential diagnosis of benign and malignant ulcer. It may be more sensitive to detect levels of HA and collagen type Ⅳ in gastic juice than those in serum. Combined determination of HA and collagen type Ⅳ levels is more valuable in diagnosis of gastric carcinoma.关键词
