Abstract
Objective To assess the diagnostic value of MRI and special physical tests for complete rupture of anterior cruciate ligament (ACL). Methods The clinical data of 88 patients diagnosed as knee ligament injury and admitted from March 2008 to February 2009 were retrospectively reviewed. Before surgery, all injured knee laxity was examined by MRI, the anterior drawer test (ADT), the Lachman test and arthroscopy. Result of arthroscopy was used as gold standard for defining ACL complete rupture. The sensitivity, specificity accuracy, positive predictive value, negative predictive value among MRI and the special physicaltests were evaluated and compared. Results There were 61 cases vith ACL complete rupture in 88 cases determined by arthroscopy. For MRI, ADT and Lachman test in diagnosis of ACL complete rupture, the sensitivity was 85.2%, 80.3% and 83.6%, respectively; specificity was 66.7%, 81.5% and 77.8%, respectively; accuracy was 79.5%, 80.7% and 81.8%, respectively;positive predictive value was 85.2%, 90.7% and 89.5%, respectively; negative predictive value was 66.7%, 64.7% and 67.7%, respectively; positive likelihood ratio was 2.56, 4.34 and 3.77, respectively and negative likelihood ratio was 0.222, 0.24 and 0.21,respectively. Conclusion Among 3 methods, ADT is of most value in diagnosis of ACL complete rupture, and the Lachman test is better in excluding the diagnosis.关键词
