双侧颈深浅丛同时阻滞的安全性及可行性探讨

摘要
Abstract
Objective To investigate the feasibility and safety of simultaneous bilateral deep and superficial cervical plexus block with 0.25％ levobupivacaine.Methods Anesthesia puncture was performed by the traditional methods of one - point deep plexus block during subtotal thyroidectomy or the radical correction of thyroid cancer.Patients undergoing bilateral block of deep and superficial cervical plexuses( group S )were injected 30 ml of 0.25％ levobupivacaine in total including 4 ml into both deep plexuses plus 3 ml into both head, foot and opposite side of superficial plexuses respectively as well as 2 ml into the both midpoints of sternocleidomastoid muscle.In group D, patients were injected 24 ml of 0.375 ％ levobupivacaine in total including 6 ml into the deep plexus plus 9 ml into the superficial plexus in the cervical side with larger lump,and only 9 ml into the superficial plexus in the other side.The anesthetic effects before anesthesia,5 and 10 min after anesthesia, during skin incision, during intraoperative exploration, and post - operation were assessed by visual analogue scale( VAS ).In addition, the complications were observed and recorded.Results The anesthetic effects of group S in which few complications were observed were better than those in group D 10 min after block,during skin incision, during intraoperative exploration, and post - operation.Conclusion The bilateral block of both deep and superficial cervical plexus with 0.25％ levobupivacaine has better anesthetic effects than traditional block anesthesia of cervical plexus.It is feasible and safe with few complications.关键词
