气管隆突支气管成形术的临床应用

Abstract
Objective To investigate the surgical indications and surgical methods of tracheal carinate bronchial plasty. Methods The tracheal carinate bronchial plasty was preformed in 33 patients with tracheal carinate bronchial tumor and 8 patients with tracheal bronchial injury. The surgical procedure performed included all or part of carinate plasty in 5, bronchial sleeve plasty in 21, tracheal bronchial plasty in 15. Results There was no operative death, but one traumatic patient had anastomotic stricture. One case of right upper lobe bronchial sleeve resection with superior vena cava replacement died from bone metastases 3 months after operation. The 1-, 3-, 5-year survival rate of the remaining 29 patients of malignant tumor were 86.2％, 55.2％,37.9％ respectively. Conclusions Tracheal carinate bronchial plasty can be applied to patients with tracheal carinate bronchial tumor or injury, to achieve both the removal of lesions, and to retain the normal lung tissue,so as to prolong survival time and improve the postoperative quality of life. It is the key to success of the choice of surgical approach, anastomosis or suture techniques, anesthesia and postoperative treatment.关键词
