连续股神经阻滞对膝部手术术后早期康复的影响
Continuous femoral nerve block promotes the early rehabilitation after knee surgery

Abstract
Objective To evaluate the effect of continuous femoral nerve block on early postoperative rehabilitation of knee joint function after knee surgery. Methods One hundred and forty patients with ASA graded Ⅰ ~ Ⅱ were enrolled in the study,including 40 cases with unilateraltotalknee arthroplasty (TKA) and 100 case with unilateralpatellar internal fixation. Patients were randomly divided into two groups (n= 70 in each): patients in group Areceived continuous intravenous analgesia and patients in group B received continuous femoralnerve block group (group B n=70). Both groups were anesthetized with generalanesthesia,group A was intravenously given loading dose of mixed analgesic 3ml (150ml mixture contains fentanyl citrate 1.2 mg + lomoxicam 40 mg) and maintained with 2ml/h, 0.5 ml for self- control each time and 20min lockout time; group B was injected with 0.15% levobupivacaine hydrochloride 5ml through the catheter as loading dose for postoperative pain relief, maintained with 4ml/h and 1h lockout time with a total prescription dose of 300ml. Postoperative analgesia therapy was lasted for 3 days. The VAS pain score, the status of out- of- bed activity and adverse reaction were evaluated and recorded. Results Compared with group A, the postoperative incidence of adverse reaction and VAS score at 1h, 6h, 24h, 48h, and 72h were significantly lower (P<0.05 or P<0.01); and patients in group B walking more ffequently and for longer distance after surgery than those in group A (P < 0.05 or P < 0.01). Conclusion Continuous femoral nerve block with 0.15% levobupivacaine hydrochloride for postoperative analgesia is beneficial for early rehabilitation after knee surgery with high safety and low adverse reaction.关键词
