克氏针张力带固定治疗儿童陈旧性肱骨外髁骨折的疗效观察

摘要
Abstract
Objective To observe initially the therapeutic result of old united lateral humeral condyle fractures in children with kirschner wire and tension. Methods From January 1995 to December 2007 ,98 cases of old united lateral humeral condylar fractures in children were treated in our hospital. All fractures were classified by Jakob classification:37 cases for type Ⅱ and 61 cases for type Ⅲ fractures. All fractures were treated by open reduction and intemal fixation with kirschner wire and tension. The purpose of this study was to observe therapeutic result and to analysis relationship between complication rate and classification. Result After a follow-up of 3 to 5 years (4.2 years on average). The time of clinical bone union was 6 to 8 weeks.According to Hardacre criteria, the overall excellent rate was 89％ ( 87/98) :95％ (35/37) for type Ⅱ and 85％(52/6l)for type Ⅲ fractures. The overall complication rate was 24. 5％ ( 24/98) , the complication rate for types Ⅱ and Ⅲ fractures were statistically significantly different ( P ＜ 0. 05 ) : 10. 8％ (4/37) for type Ⅰ and 32.8％ (20/6l)for type Ⅲ fractures. Conclusion The technology of open reduction and intemal fixation with kirschner wire and tension can effectively repair and reconstruct of the appearance and function of the elbow for old united lateral humeral condyle fractures in children. It can achieve satisfactory results in between type Jakob Ⅱ and type JakobⅢ fractures The complication rate for typeⅢ fractures was obviously higher than typeⅡ fractures.关键词
