胰腺导管腺癌中CD74的表达及临床意义
Clinical significance of CD74 expression in pancreatic ductal adenocarcinoma

Abstract
Purpose To examine CD74 expression and discuss its clinical significance in pancreatic ductal adenocarcinoma ( PDAC ).Methods 59 achival cases of PDAC and 23 of chronic pancreatitis ( CP ) were analyzed with microscopic and immunihistochemical assay. Foci of lymphocytes around nerve fibers were ohserved after hematoxylin-eosin staining and CD74 expression was examined by immunohistochemistry. Relationship between CD74 expression and clinicopathological parameters was analyzed by statistical methods.Results Foci of lymphocytes around nerve fibers was observed in 67. 8％ ( 40/59 ) of PDACs. CD74 expression was positive in cancer cells of PDACs( 61％ , 36/59 ), which was significantly higher than normal ductal epithelium in CP ( 13％ , 3/23 ) ( P = 0. 000 ).CD74 expression was significantly associated with foci lymphatic infiltration around nerves ( P = 0. 009 ), but not with other clinicopathologic parameters. Survival time of PDACs with CD74 positivity was significantly shorter than CD74-negative cases ( P = 0. 006 9 ).Cox analysis showed that CD74 expression( P = 0. 009 ; relative risk, 2. 590 ) and pT( P = 0. 022; relative risk, 1. 482 ) were independent prognositic indicators in PDAC. Conclusions The over-expression of CD74 in PDAC is related with foci lymphatic infiltration around nerves and prognosis and might be a promising target for PDAC therapy.关键词
