Abstract
Objective To investigate the clinical characteristics and treatment of severe and critically ill patients with influenza A H1N1. Method The data of 146 severe and critically ill patients with influenza A H1N1 were collected to establish a database and analyzed retrospectively. Results Logistic regression analysis:gastrointestinal symptoms on admission, body temperature, respiratory rate, rales range, LYM absolute, Plt,CK, CREA, PaO2, and lactic acid were independent factors of disease progression. There was a significant difference in the effective rate between without hormone therapy group and low dose hormone therapy group (P <0.05), no and high-dose hormone therapy group+low dose maintenance therapy group (P < 0.05 ). There was no significant difference in the effective rate between low dose hormone therapy group and high dose + low-dose maintenance treatment group (P > 0.05). For critically ill patients, there was a significant difference in the effective rate between Iow-dose group and large doses + low-dose group (P < 0.05). There was a significant difference in the effective rate between the early hormone treatment group and the late hormone treatment group (P <0.05). There was no significant difference in the effective rate between before and after7 ~ 14 days1 dexamethasone treatment (P > 0.05) , and among the three periods in 14 days dexamethasone treatment group (P > 0.05 ). The mortality was highest in invasive + noninvasive ventilation group, followed by invasive ventilation group, and lowest in non-invasive ventilation group, there was a significant difference among the three groups (P < 0.05). Conclusions Gastrointestinal symptoms on admission, body temperature, respiratory rate and the scope and rales are closely related to disease progression. Lymphocyte, platelet, creatinine, creatine kinase,partial pressure of oxygen and LAC are associated with disease progression of the disease. The application of hormones and non-invasive mechanical ventilation have a very good therapeutic effect.关键词
