Effect of the maximal tolerance dose of dose escalation on non-small cell lung cancer through 3-dimensional conformal radiation therapy

Abstract
Objective To investigate the role of maximal tolerance dose (MTD) of dose escalation in non-small cell lung cancer (NSCLC). Methods 30 patients with NSCLC with stage Ⅱ and stage Ⅲ were enrolled via dose escalation of NSCLC through 3-dimensional conformal radiation therapy. The scheduled dose escalation ranged from 68 Gy to 76 Gy.The percentage of normal lung volume which received 20 Gy was less than 37 % . When 1 case of 5-patient-group developed ≥3 grade acute reaction, this group of dose escalation would be repeated. Results 30 patients were enrolled in the test and completed the treatment. The overall survival of 1, 2, 3- year were 66.7 % , 40 %and 30 % , respectively. The median survival was 18.3 months. Conclusions Dose escalation for NSCLC through 3-dimensional conformal radiotherapy could be accepted without excessive toxicity, and may improve overall survival rates of patients with stage Ⅱ and Ⅲ NSCLC.关键词
