Abstract
OBJECTIVE: To analyze irrational medication orders of pharmacy intravenous admixture service (PIVAS) in our hospital in 2009, to enhance the awareness of rational drug use and to improve rational drug use. METHODS: 43 835 long-term medication orders of 14 wards in our hospital in 2009 were checked by pharmacists and Meikang Rational Drug Use Software. The check list mainly included improper concentration, irrational selection of solvent, drug incompatibility, excessive dosage, duplicate drug use. RESULTS: 142 irrational medication orders accounted for 0.32％. 34 irrational medication orders were caused by 6 kinds of newly introduced drugs in 2009, accounting for 23.94％. Improper concentrations (42.25％) and drug incompatibility (25.35％)took up the biggest proportion. CONCLUSION: Newly introduced drugs increase the incidence of irrational medication orders. Pharmacists should check the medication orders carefully so as to improve rational drug use in the clinic.关键词
