自发性脑干出血的临床与影像诊断研究

Abstract
Objective:To study the clinical and Imaging diagnosis value of spontancous brain stem hemorrhage, and to direct clinical therapy and prognosis. Methods:The clinical and CT and MRI examination data of 43 patients with spontaneous brain stem hemorrhage were analyzed retrospectively. Results: Among the 43 patients, 35 patients have hypertension (81.4 %) All patients were performed MRI examination. 33 cases were pontine hemorrhage. 9 cases were midbrain hemorrhage, and 1 caseswere medulla oblongata hemorrhage. 23 patients with pontine hemorrhage were performed MRI examination. 5 acute hemorrhage lesions showed isointensity signal on T1WI, and slightly hyperintensity signal On T2WI 15 subacute hematomas, 5 lesions were mixed high signal, and 10 lesions showed ring-shaped hyperintensity signal on T1WI and T2WI images 3 cases were chronic hematoma, MRI showed as high signal on T1WI and T2WI with a very low signal hemosiderin ring 38 patients were performed internal medicine treatment, 5 patients by surgery treatment. If bleeding volume less than 5ml, the patient prognosis were bctter than that of bleeding volume over 5 ml, with massive hemorrhage dicd bccause of no surgery promptly. Conclusions:Hypertension was the main cause of s brain stem hemorrhage. Most brain stem hemorrhage was located on pontine. CT was the first choice for the diagnosis of pontine hemorrhage. MRI was an additional medium of CT. CT and MRI can help the clinic to choice correct treatment method for brain stem hemorrhage, and prove the prognosis.关键词
