尼龙绳套扎在消化道大息肉方面的应用
Application of endoloop and epinephrine in treatment of digestive large polyps

Abstract
[Objective]To evaluate the effect of pure endoloop and combined with Epinephrine in the treatment of digestive large polyps.[Methods]180 endoscopic diagnosed digestive large polyps (＞2.0 cm), classified with Yamada.Stained by Lugol's solution firstly, the polyps were then biopsied in many places of basal parts, the top of polyps were received high frequency electrocoagulation based on the pigmentation.Both of tissues biopsied and resected were sent for histopathological examination.Each selected polyp was received endoscopic therapy with endoloop in the basal parts and re-examined by endoscopy one week later.Polyps not fallen off were received endoscopic therapy with endoloop for the second time and were reviewed at the second week.Polyps not fallen off after two weeks were firstly injected with saline-adrenaline injection (1∶10 000) in the basal parts and were received endoscopic therapy with endoloop again.Falling off or not for each polyp was examined at 3rd week and the polyps were followed up for 24 months.[Results]After a week of re-examination, all the 56 type Ⅳ of polyps fell off, 12 of type Ⅲ polyps fell off, 1 of type Ⅱ polyps fell off and no type Ⅰ polyps fell off.After two weeks of reexamination, 13 of type Ⅲ polyps fell off, 3 of type Ⅱ polyps fell off and still no type Ⅰ polyps fell off.At 3rd week, 39 of type Ⅲ polyps fell off, 28 of type Ⅱ polyps fell off and 23 type Ⅰ polyps fell off.There were no complications such as hemorrhage or perforation, and no new polyps or malignant changes in the positions of loop ligature after 24 months of following up.[Conclusions]The effective of endoscopic therapy with pure endoloop was good for type Ⅳ polyps, not good enough for type Ⅲ polyps and poor for type Ⅰ and type Ⅱ polyps.The effective of endoscopic therapy with pure endoloop can be enhanced obviously by injection in the basal parts of polyps with saline-adrenaline injection (1∶10 000).关键词
