摘要

Abstract

To explore the application of ELISPOT(Enzyme Linked Immunospot Assay) in the diagnosis of TB(tuberculosis) infection, TB-ELISPOT assays were performed in PBMC(peripheral blood mononuclear cell) isolated from patients who had already been clinically diagnosed as Mtb infection(54 cases) or non-TB infection(52 cases). Other diagnostic approaches,including sputum smear, sputum culture, PPD(Purified Protein Derivative Tuberculin) and DIGFA(dot immuno-gold filtration assay), were also employed for comparison of sensitivities and specificities. The data of TB-ELISPOT showed the sensitivity (Se) and specificity(Sp)were 90.7% and 96.15 % respectively. The positive predictive value(PPV) and the negative predictive value(NPV) were 0.96 and 0.91 respectively. The positive likelihood ratio(+LR ) and the negative likelihood ratio(-LR) were 23.55 and 0.1 respectively. The data of DIGFA was Se 59.26%, Sp 88.46%, PPV 0.84, NPV 0.68, +LR 5.14 and -LR 0.46. The data of PPD were Se 64.81%, Sp 82.69 %, PPV 0.80, NPV 3.74, + LR 0.69 and -LR 0.43. The data of sputum smear were Se 40.74%, Sp 100.00%, PPV1.0, NPV 0.62, +LR>10 and -LR 0.52. The data of sputum culture was Se 25.93%, Sp 100.00%, PPV 1.0, NPV 0. 57, +LR>10 and -LR 0.77. The sensitivity of TB-ELISPOT in sputum smear negative and sputum culture negative patients were 84.38% and 87.5% respectively. Results demonstrated that ELISPOT is a specific and sensitive approach in detecting Mtb active infection land latent Mtb infection. ELISPOT is a less time-consuming assay (usually 24-30 h) and could be useful in TB especially in sputum negative pulmonary tuberculosis diagnosis.
