Abstract
Objective : To evaluate of the CnRH antagonist effectiveness in vitro fertilization-embryo transfer( IV F-ET) in poor responders. Methods : CBMdisc ( 1979 - 2010 ) , Wanfang(1994 -2010) ,CNKI( 1994 - 2010) .VIP( 1989 - 2010) , PUBMED( 1997 - 2010) , PML ( 1997 -2010) , FMJS( 2000 ~ 2010) and 9 related joumals were searched for randomized controlled trails( RCTs) on the comparison between GnRH antagonist and GnRH agonist in IVF-ET in poor responders. The quality of included trials was critically appraised. RevMan4. 2. 7 software was used for statistical analysis. Results : Nine pubished studies with 731 participants were included. Compared with CnRH agonist.cancellation rate was no significant difference in two group at the WMD 1. 04 and 95％ Cl 0. 70 to 1. 55; stimulation duration in CnRH antagonist group was lower at the WMD -0. 97 and 95％ CI -1. 64 to -0. 30,the difference was statistically significant ;dose of gonadotrophins( Cns) in GnRH antagonist group was lower at the WMD -0. 49 and 95％ CI -7. 32 to 2. 48 , the difference was statistically significant; endometrial thickness at the time of HCG administration was higer at the WMD 0. 36 and 95％ CI 0. 20 to 0. 53.the difference was statistically significant; serum E2 level on the day of HCC administration was no significant difference in two group at the WMD -102. 49and 95 ％ Cl 303. 57 to 98. 59 ; number of oocytes retrieved was no significant difference in two group at the WMD O and 95％ CI-0. 5 to 0. 51 ; pregnancy rate in GnRH antagonist group was slightly higher at the WMD 1. 35 and 95 ％ CI 0. 92 to 1 99 , the difference was no statistically significant. Conclusions : Compared with GnRH agonist protocol,the GnRH antagonist protocol can obviously reduce the stimulation duration and the dose of gonadotrophins in poor responders in IVF-ET.and both clinical result is similar. The GnRH antagonist protocol provides an altemative treatment for poor responders.Because the including trails exist insufficiency on quality and quantity as well as the methodology difference,suggested that this research conclusion only serve as the reference of the alinical analysis, need rePvaluationl and renew unceasingly.关键词
