Abstract
Objective To observe the effectiveness of disease management in patients with chronic heart failure ( CHF ).Methods 146 patients with chronic heart failure of class Ⅱ - Ⅳ by New York Heart Association were enrolled in this study after their discharge from the hospital, and the patients were randomly divided into experimental group and control group.Patients in the experimental group were performed disease management for 12 weeks, while patients in control group were performed routine treatment.The effectiveness was evaluated in the 13th week after discharge.Results The utilization rate and goal dose rate of angiotensin converting enzyme inhibitor / angiotensin receptor blockers ( 55.9％ vs 28.2％ , 34.6％ vs 15.4％ , P ＜0.05 ) and β - receptor blockers ( 47.9％ vs 28.2％ , 26.6％ vs 15.4％ , P ＜0.05 ) were significantly higher in the experimental group than the control group, and the rate of the readmission was significantly lower in the experimental group than that of the control group (15％ vs32％ , P ＜ 0.05 ).The quality of life was significantly increased in the experimental group than that of the control group ( 46.17 ±4.95 vs 51.07 ± 6.33 , P ＜0.05 ).While there were no statistically significant differences in mortality between the two groups.Conclusion Clinical disease management for patients with CHF can improve evidence - based treatment, reduce the reamission rate and improve the quality of life.关键词
