Abstract
Objective To explore the clinical value of the pitch value and S/D ratio measured by umbilical aretery Doppler for prenatal diagnosis of umbilical cord torsion,and to study the clinical significance of S/D ratio for predicting the outcomes of trial production and neonates in umbilical cord torsion.Methods The prenatal diagnosis of umbilical cord torsion was established by the pitch value and S/D ratio measued by umbilical aretery Doppler.The diagnosis of umbilical cord torsion was confirmed by examining the umbilical cord after birth.According to S/D ratio, 118 pregnant women were divided into experimental group (S/D ≥ 3, n =65 ) and control group (S/D＜3, n =53 ).Th(e) diagnostic accrodance rate of umbilical cord torsion,forceps delivery rate,cesarean section rate,umbilical cord blood gas analysis,neonatal Apgar score,and characteristics of amniotic fluid were compared.Results The overall diagnostic accordance rate of the prenatal dianosis of umbilical cord torsion were 89.9％ ,and the diagnostic accordance rate was singificantly higher in experimental group than in control group (95.4％ vs 83.0％,P ＜ 0.05).Compared with control group,the rates of forceps delivery,cesarean section,and neonatal asphyxia were significantly higher in experimental group.Conclusion The pitch value and S/D ratio measured by umbilical artery Doppler could predict the umbilical cord torsion.Umbilical artery S/D ratio can be used as an indicator of intrauterine fetal status for diagnosing umbilical cord torsion and guide the choice of delivery mode.关键词
