腰椎滑脱症的翻修手术治疗和原因分析

Abstract
Objective To study the failure cause of primary lumbar spondylolisthesis operation and evaluate the revision technique and therapeutical effect.Methods 19 cases with recurrence or progression of back and leg pain following primary surgery for lumbar spondylolisthesis ( destabilization and loss of reduction in 6 cases, the loosening or breakage of implants and progressive spondylolisthesis in 13 cases ) were reoperated hy the posterior pedicle screw fixation and lumbar interbody fusion.A prospective, observational study of the clinical outcomes and radiological data was performed.Results All the cases was followed up for 5 months to 36 months.with average time of 22.5 ± 0.8 months.All cases with revision operations had good bone graft fusion.Neither loss of spondylolisthesis reduction nor loosening or breakage of pedicle screws was found.The surgical outcomes were evaluated according to HOU-Shuxun's criteria: excellent in 11cases , good in 6, fair in 1 , and poor in 1.Conclusions The main failure causes of primary lumbar spondylolisthesis operation have much close relationship with the primary operative mode, the quantity and quality of bone graft and surgical manipulation.The keys for success of rivision operation are completely decompression,ideal revision technique , rational application of internal fixation and intervertehral and posterolateral bone grafting.关键词
