尿动力学参数多元分析对良性前列腺增生所致膀胱出口梗阻诊断价值的研究
Comparative study on diagnostic value of urodynamic parameters multianalysis in the evaluation of bladder outlet obstruction resulting from benign prostatic hyperplasia

Abstract
Objective: To explore a function model which can diagnose bladder outlet obstruction ( BOO) resulting from benign prostatic hyperplasia by logistic regression analysis and allow the judgement of bladder outlet state for patients with different detrusor contractility, especially with impaired detrusor contractility; To compare diagnostic efficacy of different function models.Methods: Urodynamic examinations were performed in 336 consecutive men with lower urinary tract symptoms suggestive of benign prostatic hyperplasia.Some urodynamic parameters and the results of clinical diagnosis of BOO were analyzed by logistic analysis.Based on the analysis, function models were constructed.A cutoff point for diagnosing BOO with our function model was confirmed by ROC curve analysis.The diagnostic efficacy of different models was compared according to areas under the ROC curves.Results : We defined the function model as bladder outlet obstruction number ( BOON).The model was obtained by the formula; BOON = 2.1-0.3 × Qmax + 0.005 × RV + 0.044 × Pdetat Qmax + α ( α = 0 for those with normal pressure-normal flow on P-FS, α=1.2 for those with low pressure-low flow, α = 1.8 for those with high pressurelow flow).The threshold of diagnosing BOO with BOON was 0.6, and the sensitivity, specificity, positive predictive value, negative predictive value was 95.1％ 、 89.5％ 、 97.5％ 、 81.0％ respectively.Compared with BOOV and BOOI, BOON was highest in diagnostic efficacy and easy to calculate.Conclusion: The BOON can be calculated with an easily applicable formula composed of readily available parameters and its diagnostic efficacy was satisfactory.When differentiating BOO, BOON broke the limitation of A-G and Schafer diagram to some extent and could be used not only for those with normal detrusor contractility but for those with impaired detrusor contractility关键词
