腹腔镜与开腹肝切除术的临床研究

Abstract
Objective To compare the effects of laparoscopic hepatectomy (LH) and conventional open hepatectomy (OH). Method The clinical data of 16 cases of LH and 21 cases of OH from January 2008 to May 2010 was analyzed retrospectively. Results There was no significantly difference in age, gender,disease courses and preoperative liver function ( P ＞ 0. 05 ). All operations were successfully carried out.There was no significantly difference between two groups in the intraoperative blood loss, (597.3 ±761.8) ml vs. (511.9 ± 513.5 ) ml and the operation time, ( 144.8 ± 42.0 ) min vs. ( 136.2 ± 35.4 ) min ( P ＞ 0.05 ).And the cost of hospitalization in LH was higher than OH, ( 19 241.9 ±4 682.2) RMB vs. ( 14 229.7 ±3 805.4) RMB ( P ＜ 0.05 ). But it decreased significantly in the LH group in the mean time of bowel function recovery, ( 50.6 ± 7.9 ) h vs. ( 69. 1 ± 8.3 ) h ( P ＜ 0.01 ) , the total abdominal drainage volume ( 153.1 ± 106.7 ) ml vs. ( 260.7 ± 188.3 ) ml ( P ＜0.05 ), and the postoperative hospitalization (9.3 ± 2.2) days vs. ( 12. 1 ± 3.9) days ( P ＜ 0.05 ). Patients in LH group recovered sooner in liver function and achieved higher primary intention healing rate of their incision. Conclusion ff suitable case is selected, LH is a feasible and safe therapy with advantage of less pain and minimal invasiveness.关键词
