Abstract
Objective To evaluate the diagnostic accuracy of CTE, including computed tomography enteroclysis and computed tornngraphy enterography, and other modalities such as capsule endoscope (CE), MR enterography (MRE) and SBFT in Crohn's disease (CD). Methods Following PICO, related literature was fully retrieved from MEDLINE (1960-2010), PUBMED, EMBASE, CCTR and Cochrane Library combined with manual search. The prospective or retrospective studies which evaluated or compared the diagnostic accuracy of CTE and CE, SBFT, MRE were included. QUADAS items were used to evaluate the quality of the included studies. SEN,SPE, +LR, -LR and the areas under SROC were detected. Results Nine trials including 360 cases met the inclusion criteria. The reference standard included endoscopic, histological and radiological data, imaging methods (①) and ileocolonoscopy+ biopsy or operative findings (②). There were four and five research projects conforming to these two reference standards (①) and ②), respectively. SEN values were 0. 77 (95%CI: 0. 69-0. 84), 0. 86 (95%CI:0. 80-0. 90); SPE values were 0. 81 (95%CI: 0. 72-0. 88), 0. 97 (95%CI: 0. 89-1.00) ; +LR values were 3. 44 (95%CI: 1.61-7.32), 14. 67 (95%CI: 5.31-40. 50); -LR values were 0. 30 (95%CI.. 0. 18-0. 50), 0. 16 (95%CI:0. 11-0.22). Areas under the SRO(C curve were 0. 8762 and 0. 8429, respectively. In CTE, the sensitivity plus specificity degrees were greatest, and the next was SBFT, followed by MRE, and CE was the last. Conclusion CTE is superior to all other modalities in the diagnosis of small bowel Crohrns disease.关键词
