摘要

Abstract

Purpose: One hundred percent O2 is used routinely for preoxygenation and induction of anesthesia. The higher the O2 concentration the faster is the development of atelectssis, an important cause of impaired pulmonary gas exchange during general anesthesia (GA). We evaluated the effect of ventilation with 0. 5 FiO2 in air and 100% O2 following intubation on the development of impaired gas exchange. Methods: One hundred and two patients aged 1 month - 15 yr, undergoing cardiac surgical intervention for congenital heart disease(non- cyanosis ) were administered 100% O2 for preoxygenation (three minutes) and ventilation by mask ( two minutes). Following intubation these patients were randomly divided into two groups of and ventilated either with0.5 FiO2 in air of 49 cases or 100% O2 of 53 cases. Which in before preoxygenation 、30 min following intubation 、entering ICU、after extubation 30min and three days of postoperation, afterial blood gases were obtained for analysis. Subsequently PaO2/FiO2 ratios were calculated. Results were analyzed with Student's t test and one - way ANOVA. P value of < or = 0.05 was considered significant. Results: PCO2 value in entering ICU (C) that was significantly improved 0. 5 FiO2 group than 100% O2 group. PaO2 value in entering ICU(C) and after extubation 30min(D) that were significantly improved 0.5 FIO2 group than 100% O2 group. PaO2/FiO2 ratio value of 0.5 FiO2 group were more improved in after intubation 30min(B), in entering ICU(C) and after extubation 30min(D). Ventilation of the lungs with O2 in air0.5 FiO2 significantly improved the PaO2/FiO2 ratio(P < 0.05). Conclusion: In cardiac surgical intervention for congenital heart disease (non - cyanosis ) children during GA, ventilation of lungs with O2 in air0.5 FiO2 was significantly improved gas exchange than in 100% O2.
