Composite skin transplantation with allogeneic epidermal stem cells and porcine acellular dermal in six cases

摘要

Abstract

Six patients with deep bum were from Department of Burn,the First Affiliated Hospital of Nanchang University between January 2005 and October 2006.They were all Ⅲ degree burn of limbs.Composite skin made with xenogenic(porcine)acellular dermal and allogeneic epidermal stem cells was provided by Burn Center in the First Affiliated Hospital of Nanchang University.After debridement and sterilization,composite skin transplantation and autologous split-thickness skin transplantation were performed.A total of 96 autologous split-thickness skins and 12 composite skins were transplanted.In composite skin transplantation,the composite skin was cut into 1.5 cm×1.5 cm and transplanted on the wound with rough surface opposite to wound.In autologous split-thickness skin transplantation,autologous split-thickness skin placed on vaseline gauze and cut into 1.5 cm×1.5 cm,and then transplanted around of the composite skin.At 3 days after transplantation,the composite skin and autologous split-thickness skin adhered well with wounds,which showed pink and wet.At 7 days,the majority of the composite skin and autologous split-thickness skin were survival.At 14 days,the majority of skins were close to fusion.At 21 days,the wound had basically healed.There was no significant difference in the number of skin graft survival between the composite skin and autologous split-thickness skin(P>0.05).But the healing area of the composite skin was significantly smaller than that of autologous split-thickness skin because the composite skin had the poor extension(P<0.01).There was no patient had bad reaction.The results suggested that the artificial skin made with epidermal stem cells and porcine acellular dermal transplanted to the affected areas could establish blood circulation and maintain a certain degree of vitality,which could achieve the purpose of covering the wound,preventing infection and preventing the loss of body fluids.
