四联手术治疗难治性青光眼合并白内障疗效分析

Abstract
Objective To evaluate the therapeutic effects and safety of combined phacoemulsification, foldable intraocular lens implantation, trabeculectomy and glaucoma drainage device implants ("Tetra- combination surgery") for patients with cataract and primary angle closure glaucoma.Methods The tetra- combination surgery was performed through a 3.5 mm incision by means of conventional scleral flap on 68 eyes of 56 patients with cataract and primary angle closure glaucoma,after follow- up of l, 3, 6, 12 and 24 months the changes of intraocular pressure (IOP), filtering blebs, pre- operative PAS range and corrected vision of patients were observed.Results During the follow- up the mean intraocular pressure (IOP) vas decreased after surgery (P＜0.01).The filtering blebs of type Ⅰ and Ⅱ by Kronfeld's classification presented in 86.76％ ofthe eyes and type Ⅲ and Ⅳ in 13.24％ ofthe eyes.The pre- operative PAS range degree reduced to ≤ 90° (P＜0.05)in 46 eyes (67.6％).After follow- up for 12 and 24 months the corrected vision was ≥ 0.5 in 45 eyes (66.2％) and 52 eyes (76.5％) respectively.Conclusion The tetra- conbination surgery is an effective, quick, repeatable and safe procedure for patients with cataract and intractable glaucoma.The surgery can not only increase the vision but also reduce the intraocular pressure (IOP) effectively, prolong the function of filtering blebs, increase the rate of success of filtering operation.关键词
