单操作孔全胸腔镜自发性气胸的手术治疗

Abstract
Objective To discuss the clinical feasibility of the surgical treatment of spontaneous pneumothorax by uniportal video - assisted thoracic surgery ( VATS ), and to summarize clinical experiences. Methods The clinical data of 61 cases of spontaneous pneumothorax in our department were retrospectively analyzed. All cases were treated with uniportal VATS. Results The average operation time was 55( 20 - 145 )min. The average perioperative blood loss was 60( 10 - 400 )ml. The average postoperative total drainage was 280( 50 - 1600 )ml,and average retention time of intrathoracic drain tube was 2. 5( 1 - 10 )d. The average postoperative length of stay was 5. 5( 2 - 12 )d. All patients recovered smoothly without severe complications except for one case who received re -operation because of prolonged postoperative pulmonary air leakage. Follow - ups were made 1 to 20 months after the discharge, and there was one case of palindromia who was cured by thoracic close drainage. Conclusion It is safe and feasible to treat the spontaneous pneumothorax with uniportal VATS. This method can further decrease the trauma and its curative effects are similar to that of the traditional three - portal VAST. It should be used as the main standard treatment for spontaneous pneumothorax.关键词
