Reproductive Effect of Metformin in Patients with Polycystic Ovary Syndrome Undergoing IVF/ICSI:a Systematic Review

Abstract
Objective:This review systematically evaluated clinical value of metformin in patients with poly-cystic ovary syndrome (PCOS) undergoing IVF/ICSI. Methods: Electronic search of the MEDLINE, EM-BASE, Cochrane Library, Chinese Biomedical Database, VIP Information, China National Knowledge Infrastructure (CNKI) and Wanfang database was performed to identify randomized controlled trials or randomized cross-over trials assessing metformin in patients with PCOS undergoing IVF/ICSI up to February 2011. Quality assessment and data extraction were performed independently by two reviewers. Data was combined for meta-analysis using M-H odds ratio for dichotomous data or mean difference for continuous data. Results; All 9 trials (11 articles) were included finally. Meta analysis showed that in patients with PCOS undergoing IVF/ICSI, metformin group significantly had more clinical pregnancy events than placebo group (OR = 1. 82, 95% Cl 1. 28 ~ 2. 59), less OHSS ( OR = 0. 39, 95% Cl 0.21 - 0. 70), more number of embryos transferred (MD = 0.12,95% Cl 0. 03 ~ 0.21), and more patients reported adverse effects ( OR = 9.49, 95% Cl 4.10 -21. 97). But there were no differences between metformin and placebo group in live birth rate, days of ovary stimulation, total dose of rFSH, serum E2, number of oocytes collected, fertilization rate, cycles cancelled and some biochemical outcomes. Conclusions:Metformin may increase clinical pregnancy events and transferred embryos, but could not improve the quality of oocyte.关键词
