Abstract

Objective To investigate the value of spiral CT and multi-plane reconstruction (MPR) in guiding ear canal and middle ear reconstruction for patients with congenital microtia. Methods Patients with Schuknecht type C congenital microtia (32 ears) were examined with spiral CT, using continues volume scanning. Axial and coronal images were acquired to determine the route of facial nerve. Images of inclined axial and coronal planes were also obtained on an image work station, allowing viewing of middle ear and cribriform areas on the same image and appropriate display of axial and anteroposterior angles. The images were then used to guide reconstruction surgeries for these patients. Results With image guidance, location of the middle ear was successful in 29 of the 32 ears (90.6%) and failed in 3 ears (9.4%) due to poor pneumatolysis of the mastoid and malformation of the facial nerve. In contrast, in the same procedures performed on 41 ears of similar congenital microtia without MPR guidance, location of the middle ear was successful in only 28 ears (68.3%) and failed in 13 ears (31.7%). The difference was statistically significant (Chi-Square Test). In addition, the mean operation time when using MPR guidance was 342.75 ± 41.70 min (n=28), compared to 398.48 ± 61.02 min (n=23) without MPR guidance. Again the difference was statistically significant (t-test). Conclusion HRCT and MPR provide images of any desired angles after a single scan, useful in demonstrating anatomy for ear canal and middle ear reconstruction. They may help improve surgery outcomes and shorten operation time. HRCT and MPR can be of important value in surgical treatment of congenital microtia.
