胃肠道间质瘤294例临床病理分析

Abstract
Objective To study the clinicopathological and immunohistochemcal characteristics of gastrointestinal stromal tumor (GISTs). Methods Clinical and pathological data about 294 GISTs patients admitted to Chinese PLA General Hospital from 2006-2010 were analyzed. Results The number of GISTs patients tended to increase year by year. The male and female ratio was 1.35:1. The age of GISTs patients ranged 22-81 years(mean 56 years). The clinical manifestations of GISTs were gastrointestinal bleeding, anemia, abdominal pain or distension, abdominal discomfort and masses, and 32.7% patients had no symptoms. GISTs was located in esophagus and cardia of 6 cases, in stomach of 159 cases, in small bowel of 100 cases, in colon and rectum of 12 cases, and in other peritoneal sites of 17 cases. Immunohistochemistry showed that the positive expression rate of CD117 and CD34 was 87.9% and 86%, respectively. CD117 and CD34 were both positively expressed in 75.9% GISTs patients. Of the 294 GISTs patients, 238 received surgical treatment, 36 received laparoscopic surgery, 1 received combined endoscopic surgery, 2 received combined endoscopic and laparoscopic surgery, and 7 underwent endoscopic excision, ligation or endoscopic sub-mucous decollement, 10 received no therapy or only biopsy. Conclusion GISTs predominantly occurs mainly in stomach and small bowel of middle- and old-aged individuals with a slightly higher incidence in males than in females. It has no specific symptom. Combined CD117 and CD34 detection can increase its accurate diagnosis rate, and surgical resection is the most effective procedure for it.关键词
