Efficacy Observation of Amphoterincin B Combined with Anti-tuberculosis Agents for Tuberculous Meningifts Complicating with Cryptococcal Neoformans Meningitis

Abstract
OBJECTIVE: To observe the clinical efficacy of amphoterincin B combined with anti-tuberculosis agents for tuberculous meningitis complicating with cryptococcal neoformans meningitis. METHODS: 52 tuberculous meningitis patients with cryp-tococcal neoformans meningitis were randomly divided into treatment group and control group with each group of 26 cases. Both 2 groups were given therapy of anti-tuberculosis, lowering intracranial hypertension, glucorticoids, hepatoprotectives and heteropathy. Treatment group was additionally given Amphoterincin B injection 1 mg·kg-1·d-1 as initial dose and then escalated dose gradually; Control group was additionally given double initial dose of fluconazole injection 200 mg·kg-1·d-1. During the 45 days of treatment, protein of cerebrospinal fluid was reviewed every week. RESULTS: Protein of cerebrospinal fluid went down to normal in 84.6% patients of treatment group, while in 57.7% of control group. There was significant difference (P<0.05). Adverse drug reaction appeared in 5 cases of treatment group (19.2%) and 4 cases of control group (15.4% ). There was no significant difference (P>0.05). CONCLUSION: Amphoterincin B combined with anti-tuberculosis agents for tuberculous meningitis complicating with cryptococcal neoformans meningitis could effectively lower the protein of cerebrospinal fluid. Adverse drug reaction was small and low.关键词
