腹腔镜辅助下进展期胃癌D2根治术的临床分析

Abstract
Objective To study the feasibility and recent efficacy of laparoscopic assisted D2 radical gas-trectomy for the progressed gastric carcinoma. Method To analyze 30 cases that experienced laparoscopic assisted D2 radical gastrectomy for the progressed gastric carcinoma, including 14 cases of total gastrectomy, 16 cases of distal subtotal gastrectomy. Results The laparoscopic surgery was done smoothly on 30 patients, no conversion to open surgery. The average surgery time was (261.2 ± 22.1) min of total gastrectomy and (239.8 ±19.7) min of distal gastrectomy. The mean intraoperative bleeding quantity was (133. 3 ± 30. 2) ml of total gastrectomy and (110. 4±27.5)ml of distal gastrectomy. The mean quantity of lymph node dissection was (21.6 ±2.3). Postoperative gastrointestinal function recover time was(3.7 ± 1.1)d,and the mean off-bed time was (2.6 ±0.5)d. Good efficacy was achieved after surgery and no recent complication occurred. All the patients got follow-up for 3 to 36 months. 2 cases got liver metastasis and 1 case got tumor recurrence. No occurrence of trocar deposit or peritoneal metastasis. Conclusion Laparoscopic D2 radical gastrectomy for gastric carcinoma has the merits of small wound, fast recovery, safety and good recent therapeutic effect.关键词
