rAd-p53治疗前后鼻咽癌原发灶CD34标记的MVD差异及意义
Difference of the CD34 Marked Microvessel Density in Nasopharyngeal Carcinoma before and after rAd-p53 Intratumoral Injection and its Significance

Abstract
To investigate the difference of CD34 marked microvessel density( MVD )in nasopharyngeal carcinoma before and after the recombinant human p53 adenovirus( rAd-p53 )intratumoral injection, and to investigate its significance. Methods 63 patients with advanced nasopharyngeal carcinoma were randomly divided into two groups. Treatment group( 32 cases ):rAd-p53 intratumoral injection and concurrent chemo-radiotherapy, control group( 31 cases ): concurrent chemo-radiotherapy. The clinical data and the nasopharyngeal tumor tissues which were treated by intratumoral injection with rAd-p53 were collected. The two-step immunohistochemical method was used to detect MVD, which was marked by CD34 in the primary tumor tissue of nasopharyngeal carcinoma. Results The MVD was not significant difference between treatment group and conventional therapy group before treatment( P > 0. 05 ). The MVD of treatment group was 18.0 ±9.22 before rAd-p53 intratumoral injection,while the MVD were 13.63 ±9.93 after treatment,the difference was significant ( P <0.05 ). Incontrol group,the MVD were 19. 39 ± 12. 43 before chemo-radiotherapy,while the MVD were 17.96 ±9.53 after treatment, there was no significant difference( P >0. 05 ). The rate of relapse and metastasis were 10. 81%( 4/37 )in MVD decreasing group,38. 46%( 10/26 )in MVD increasing group,respectively, the difference was significant P <0.05 ),while the rate of metastasis were 5.41%( 2/37 )and 26. 92%( 7/26 ),respectively,the difference was significant P < 0. 05 ). Conclusion CD34 may be associated with distant metastasis, the expressions of CD34 protein and MVD decrease in patients with nasopharyngeal carcinoma that received intratumoral injection of rAd-p53, so as to reduce the risk of distant metastasis. rAd-p53 intratumoral injection may prevent distant metastasis for patients with advanced nasopharyngeal carcinoma,especially for patients with occurrence of cervical lymph node metastasis.关键词
